Job Summary:

The Palliative Care Coordinator is crucial part of the day-to-day operations of the palliative care program. In this role you will be responsible for coordination with referral sources, vendors, patients, families, and internal staff regarding the care provided to ensure the highest level of growth, quality care and customer service.

Qualifications:

• Minimum of 3 years Administrative Assistant; BS degree is preferred but not required.
• Minimum of (3) years working in an administrative role.
• Excellent written and verbal communication skills. Microsoft Office,
• Excel and database experience required.
• Knowledge of hospice and palliative care concepts, practices, and procedures preferred.
• Understands and demonstrates customer service philosophy.
• Exceptional organization skills
