POSITION DUTIES In Patient Kaiser MSW discharge planning preferred.

MINIMUM REQUIRED QUALIFICATIONS DCP Experience

PREFERRED QUALIFICATIONS Knowledge of Health Connect/Epic preferred.

LENGTH OF ASSIGNMENT 13 weeks

SHIFT / HOURS PER WEEK Schedule: Week 1 Mon-Tues-Sat

Week 2: Sun-Mon-Tues-Wed-Thurs

32 GWW - 8 hour shifts days

START DATE 5/19/25
