Job Description

Job Title: RN Care Manager

Job Summary:

As an RN Care Manager, you will be responsible for coordinating patient care within a healthcare setting. You will assess patient needs, develop care plans, and ensure that the appropriate services are being provided to meet those needs. You will also work closely with other healthcare professionals to ensure continuity of care and promote optimal patient outcomes.

Responsibilities and Duties:

1. Conduct patient assessments to determine individual care needs.

2. Develop and implement care plans based on patient assessments.

3. Coordinate with healthcare team members to ensure that care plans are being followed.

4. Provide education and support to patients and families regarding their healthcare needs.

5. Monitor patient progress and make adjustments to care plans as needed.

6. Advocate for patients to ensure they receive the appropriate services and resources.

7. Maintain accurate and up-to-date patient records.

8. Participate in case conferences and multidisciplinary meetings.

Qualifications and Skills:

1. Active RN license in the state of practice.

2. Bachelor's or master's degree in nursing.

3. Previous experience in care management or case management preferred.

4. Strong communication and interpersonal skills.

5. Ability to work effectively in a team environment.

6. Knowledge of healthcare systems and resources.

7. Excellent organizational and time management skills.

8. Critical thinking and problem-solving abilities.
