About Job

Position Summary:

Beautiful Minds is a CSA designed to provide clinical support to individuals with mental health disorders who need services. The program is intended to provide resources to meet all of an individual's mental health needs, including psychiatric services, community support, and counseling care. This position provides psychosocial assessments and makes referrals to clients in the community as necessary. Responsible for coordinating referrals and treatment between clients and the treatment team.


Minimum Qualifications:

  • Master's Degree in Counseling, Social Work, Psychology, or a related field preferred.
  • Two years of professional experience in mental health.
  • Must have reliable transportation, liability insurance, and a driver's license.

Preferred Qualifications:

  • District of Columbia professional license as Mental Health Counselor, Clinical Social Worker preferred, or license-eligible.
  • LGPC or LGSW will be supervised by licensed staff.

Special Skills, Knowledge, and Abilities:

  • Able to create a culture of respect and adapt treatment in response to the client's gender, etc.
  • Excellent verbal and written communication skills.
  • Familiarity with evidence-based/informed treatment models.
  • Professional experience working in crises.
  • General understanding of case management and referral duties.
  • Able to work independently in a self-motivated multi-discipline team environment.
  • Good organizational skills, analytic ability, and attention to detail.

Essential Functions/Job Duties:

  • Independently manage referrals, keep track of enrollees, and the length of time between enrollment and completion of the intake process.
  • Participate in the intake/assessment process to formulate a diagnosis, psychosocial evaluation, and Individual Care Plan (ICP). LGPC/LGSW or those unable to formulate a diagnosis will be under the supervision of licensed staff.
  • Develop/monitor ICPs with clients and collaborate with the psychiatrist and treatment team. Demonstrate good clinical judgment in decision-making and cultural awareness/sensitivity in working with individuals from diverse backgrounds with complex treatment needs.
  • Meet with clients as scheduled to aid in the development of therapeutic relationships.
  • Refer, advocate, and link clients to additional services and resources in the community as needed. Act as a liaison and consult with community agencies to maintain coordination during the intake process.
  • Assign clients to the management team upon the upcoming completion of the intake process. Track clients until the first contact is made by the assigned worker or manager.
  • Assist with updating enrollment tracking sheets. Conduct follow-up calls to clients to remind them of intake appointments and/or cancellations. Discharge clients in both systems (Beautiful Minds and DBH) who did not complete the intake process.
  • Assist with training new therapists in understanding the DA process, which includes updating the clients' DAs, Treatment Plans, Gains, and Locus every six months.
  • Assist as needed in updating clients' DAs, Treatment Plans, Gains, and Locus. Keep track of clients who are only receiving medication management and update DAs and Treatment Plans when they expire.
  • Manage data and provide periodic reports on encounters, demographics, kept appointments, cancellations, etc.
  • Engage clients directly to coordinate care and direct them accordingly.
  • Provide timely follow-up in documentation and responsibilities related to the clients.
  • Ensure the privacy and security of protected health information per HIPAA requirements.

Other:

  • Must meet state criminal background check requirements.
  • Must maintain employment eligibility status.
  • Attend staff meetings, weekly supervision, work with community partners, report, follow contract policies and processes, etc.

Travel:

  • Might be required to meet clients in the community if unable to attend appointments in the office setting.
