As a key mentor and guide, you’ll provide personalized support to participants, working alongside their families to ensure success in high school and beyond. From navigating transitions to high school to crafting a solid post-graduation plan, you’ll play a vital role in shaping bright futures.
KEY RESPONSIBILITIES:
Immediate Community Relationships
- Provide transition activities for middle school Girls on the Rise participants.
- Work closely with participants’ parents and families to establish trusting relationships and to orient participants and guide them through the transition program.
- Establish relationships with local schools (elementary/middle, secondary and colleges)
- Maintain relationships with relevant high school personnel to ensure participants are successful; troubleshoot obstacles.
- Establish relationships with other community supports to assist in post-secondary transition to school or work.
- Maintain data on all program activities using the transition database.
High School Transition
- In conjunction with Girls on the Rise program staff and Transition Coordinator, begin high school transition activities with 6 graders: host career days, arrange tours, attend open houses, host guest speakers.
- Discuss high school goals and choices with middle school participants and their families.
- Help guide middle school participants and their families through high school applications, charter school registration, financial aid and enrollment.
- Recruit volunteers to assist with high school application support.
- Assist middle school participants with high school placement by conducting mock entrance interviews, scheduling shadow days, and providing preparation for placement exams.
- Work with Education Coordinator to provide High School Placement Test Preparation.
- In conjunction with Girls on the Rise staff, facilitate discussions with transition groups about transition process, feelings about change, potential transition obstacles and solutions.
High School Success
- Work in collaboration with Girls on the Rise staff to engage high school students and provide mentoring, coaching, and academic and social support.
- Collaborate with parents, students, high school personnel and other community-based support to address transition obstacles and provide solutions.
- Begin goal setting for post-graduation years (e.g., higher education, formalized job training, employment).
- Provide test preparation for SAT and ACT; utilize volunteers and community partners.
- Provide high school course selection advice in support of post-graduation goals.
- Maintain close contact with all high school participants, their families, counselors and teachers, including making regular school site visits.
- Provide or procure individualized support services including tutoring, homework assistance, mentoring and service opportunities, as needed.
- Encourage engagement in positive extracurricular activities.
- Expose high school participants to higher education and career opportunities.
- Maintain monthly contacts with high school participants.
- Maintain monthly contacts, or more as needed, with parents and families.
Post-Graduation Planning and Support
- Develop, in collaboration with participants and families, a post-graduation plan that includes goals, objectives, activities, personnel and timelines.
- Identify and implement necessary supports to attain goals.
- Guide participants through the application process commensurate with their post-graduation plan.
- Assist with employment, training and college applications and financial aid forms.
- Collaborate with high school counseling and student services staff.
- Utilize Christ Child members/volunteers as appropriate to assist with transition activities (e.g., establishing long-range partnerships with employers and educational institutions).
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED
Excellent organization, communication, and problem-solving skills with the ability to prioritize assignments and meet deadlines on multiple tasks.
- Ability to cultivate and maintain positive, supportive relationships with youth and families.
- Excellent interpersonal, verbal and written communication skills.
- Proficient with Microsoft Office, databases, and Internet research.
- Minimum three years’ work experience, preferably in outreach and program capacity.
- A bachelor’s degree in social work, counseling, education, community outreach, or a related field is required.
