Job Overview
We are seeking a Case Manager for our rapidly growing non-medical home care agency. The ideal candidate will have a mix of sales and experience managing people. This is NOT a nursing case manager role.

Responsibilities

• Conduct thorough patient intakes to determine individual needs and develop personalized care plans.
• Recruit caregivers and assist in the hiring process
• Market to potential private pay and clients on the medicaid homecare waiver program
• Accurately complete and document patients needs to share with caregivers

Requirements

• Comfortable initiating conversations with strangers
• Strong organizational skills
• Ability to manage employees in a challenging enviroment
