• Excellent communication, time management, and organizational skills
• High school graduate or equivalent with one year experience in a healthcare environment.

Responsibilities

Below is a list of your responsibilities as a Care Coordinator with YesCare at our location on State Road.

• Manages daily processes related to the collection and documentation of patient appointment refusals
• Assists in collecting and managing data related to access to care initiatives as assigned
• Reporting access to care issues to the site leaders and security.
• Ensures appropriate security measures are followed as outlined by the facility and necessary data is tracked.
