Shift: Full time Days

Job Description:

Provides specialized Social Work needs in relation to medication assistance and compliance. Will collaborate with the Care Progression team to identify patients who are uninsured or underinsured in need of resources and work with these patients obtain life-sustaining medications through various resources. Collaboration with patients and community resources to increase compliance with medications, improving a patient’s overall health. The Medication Assistance Social Worker functions as a member of a multidisciplinary team for patient care. Duties may also include assistance with program development, evaluation, professional education, or supervision of students and BSW staff.

Master’s degree in social work from a school of social work accredited by the Council on Social Work Education (CSWE).

Knowledge of theory and practice of social work at the MSW level and have basic research skills.

Must have the ability to work as part of a multidisciplinary team and with patients of various ages and cultural backgrounds.

Must have excellent written/verbal communication skills.

EXPERIENCE: Prefer prior experience in a healthcare setting (i.e., hospital, home health or nursing home) but not required.

SPECIAL SKILLS: Excellent communication, interpersonal, organizational and presentation skills.

OTHER: Computer skills, Basic typing skills

EDUCATION: Required:

LMSW or LCSW

Missouri Social Work license

Master's Degree in Social Work
