GENERAL DESCRIPTION

The Behavioral Health Care Coordinator is responsible for scheduling, organizing and managing several aspects of a patient’s healthcare maintenance and treatment in the behavioral health department. Their duties include helping patients’ complete paperwork, communicating with a healthcare team about a patient’s treatment needs and educating patients about resources. This position assists with the ACT (DUI) and Anger Management programs and client registrations. This position assists all counselors in the Behavioral Health Department with requests.
DUTIES, ROLES AND RESPONSIBILITIES
· Provide comprehensive care management, care coordination and referral to community supports as needed.
· Develop and disseminate reports related to integrated behavioral health and related programming as assigned.
· Track patient related data for behavioral health clients.
· Maintain a waitlist of referrals for behavioral health services.
· Consult with team about behavioral health needs of patients.
· Collect information from individuals that identify risk factors, provide referrals and help with facilitation of care.
· Input screening results in to electronic health records and notify providers.
· Develop and manage patient care panels in an integrated setting.
· Practices according to clinic policies, procedures, and protocols, and practice guidelines to maintain a safe environment for patients, visitors, and staff
· Facilitate interdisciplinary communication.
· Utilize other members of the team in assessing and analyzing data.
· Participate in staff organizational meetings.
· Maintain core competencies in relation to working with co-occurring disorders through continuing education and implementing skills into all aspects of treatment.
· Ensure client charts are maintained in a secure environment and prepared for auditing and/or archiving as needed
· Ensure all activities are performed with the utmost attention to patient confidentiality and HIPAA
· Responsible for maintaining ACT/PFL files and data entry into the ACT/PFL spreadsheets and other assistance for this program as required.
· Assist in the anger management program as required.
· Assist with the local Behavioral Health Court as required.
· Promote and support HIA through other activities in fundraising, professional representation, public appearances
· Attend monthly staff meetings and education and training opportunities
· Additional duties as assigned by supervisor and/or Executive Director
KNOWLEDGE, SKILLS, ABILIITES, AND USE OF EQUIPMENT
· Ability to assess available resources to provide patient care.
· Establish working relationships with staff, patients/families and public.
· Knowledge of general clinical practices.
· Uses relevant evidence-based practice for patient outcomes.
· Performs other special projects and tasks as assigned.
· Maintains confidentiality of patient, institutional and staff information.
· Adheres to professional and legal standards of behavioral Health Practice
· Participates in meetings and projects.
· Analyzes own performance in accordance with position expectations, develops goals and implements plan to meet those goals.
· Is self-directed in utilizing time
· Ability to utilize EHR, office suite and grant management programs
QUALIFICATIONS, EDUCATION, TRAINING, AND EXPERIENCE (REQUIRED OR DESIRED)
· Bachelor’s Degree in social work or counseling related field is preferred.
· May consider a combined work and education experience of at least two years
· Current certification in CPR with AED. May complete within 3 months of employment
· Valid Montana Driver’s License with a driving record that will allow coverage on the center’s insurance policy.
- Good understanding of Native American cultural competence
· Current certification in CPR with AED. May complete within 3 months of employment
PHYSICAL DEMANDS
Work is performed in a medical and behavioral health office and clinic setting; sitting, with intermittent standing and walks; reaches for and uses writing instruments and keyboard; reads reports and other written materials; extensive use of telephone and oral communication with the public and coworkers; stoops, bends, kneels, reaches for, picks up, and pushes or pulls; ability to lift up to 25 pounds.
WORKING ENVIRONMENT
All employees of Helena Indian Alliance may have contact with patients, families, visitors that may be physically debilitated, have mental or psychological conditions, or may be under the influence of a substance. Employees work areas include the entire facility and its property. Employees will have interaction with the entire staff throughout the facility. The position will have regular communication with outside agencies and facilities related to field of work.
