We are a Home Care agency seeking an up-beat, compassionate, and professional Service Coordinator/SCHEDULER WITH HOME CARE EXPERIENCE to join our team , in the Northeast Philadelphia area.
Job Summary:
- Scheduling agency home health care aides accordingly and ensuring agency aides work their scheduled shift.
- Managing last minute calls outs, and finding appropriate coverage.
- Developing an on-call pool of agency aides with various shifts in coordination with Human Resources.
- Communicate with referral sources and Case Managers to provide an excellent customer service experience
- Listening to needs of clients and matching them to the appropriate caregiver(s)
- Manage Caseload of up to 100 patients
- Review the care plan with patients and caregiver
- Essential Functions and Responsibilities:
- Creating and maintaining client and CAREGiver schedules with an emphasis on high quality care and the development of extraordinary relationships
- Being able to review care plans of new cases with both the clients and the CAREgivers making sure it is well understood before service starts.
- Must be a people person when it comes to resolving conflicts, clients complaints and grievances when it does arise.
- Managing last minute call outs and finding appropriate coverage.
- Maintains adequate numbers of available on-call caregivers.
- Ability to effectively communicate (in person, by email, and by telephone) with staff, clients, family members; good interpersonal relations
- Maintain absolute confidentiality of all information pertaining to employees, clients and client’s family members
- Perform general clerical duties – prepare correspondence, copying, filing, scanning, uploading, shredding and distribution and timely copying and filing of appropriate caregiver and client documentation
- Create and update client and caregiver records to ensure compliance with company policies and procedures
- Maintain organized documentation for caregivers, client files, and payroll

Providing coordinated care to patients by developing, monitoring, and evaluating their home health care plans.
- Ensure a high level of care for the patient is given by the home health aides.
- Scheduling agency home health care aides accordingly and ensuring agency aides work their scheduled shift.

- Developing an on-call pool of agency aides with various shifts in coordination with Human Resources.

- Resolve caregiver and client grievances and complaints
- Address over utilization of hours
- Reporting personnel performance issues
- Ensure caseload retention
- Contribute to team efforts by accomplishing related results as needed
Minimum Qualifications:

High school graduate (Associate or higher preferred)
- Demonstrated capability maintaining strict confidentiality
- Proven ability to generate leads and monitor referrals
- Strong typing and computer skills
- Well organized, accurate, and attentive to detail
- Excellent communication, public relations and follow up skills
- Experience with HHA Exchange and EVV
- Must Speak Fluent English.
Job Type: Full-time
Pay: $13.00 - $14.00 per hour
