The NorthPoint Health and Wellness Behavioral Health Clinic is hiring two Intake Coordinators who will be responsible for managing all referrals made to the Behavioral Health department. These coordinators will be responsible for following up on each referral received, completing an intake for behavioral health services, completing registration and insurance activities, scheduling appointments, and documenting all services provided. These coordinators will also engage in outreach to those residing in North Minneapolis and surrounding communities about behavioral health services.

Location and hours:
These positions are located at the NorthPoint Health and Wellness Center, 2220 Plymouth Avenue North, Minneapolis, Minnesota. Work hours are Monday through Friday, between 8:30 a.m.– 5 p.m.
About these position types:
These are full time, benefit earning positions. These positions are internally classified as a Case Management Assistants.

In these positions, you will:
- Utilize electronic health records (EHR) to register patients, check insurance, complete behavioral health intakes, and appropriately document all patient-provided information.
- Match patients with behavioral health providers and schedule appointments.
- Conduct behavioral health outreach to represent NorthPoint Behavioral Health at community events.
- Prepare and maintain records and reports related to outreach and intake.
- One of the following:
- Two or more years of responsible, paid social service experience providing direct client service.
- A two-year human services certificate.
- Two or more years of college coursework in the behavioral sciences field.
- Experience:
- Registering patients, checking insurance, completing behavioral health intakes, and appropriately documenting all patient-provided information.
- Public speaking and connecting with various audiences.
- Gathering information or completing the risk assessment using social work interview techniques.
- Taking initiative with demonstrated ability to work independently and as a team member.
- With relationship building, time management and leadership skills.
- Knowledge of:
- Hennepin County Health and Human Service resources.
- Current standards and techniques of social work practices including all applicable laws, rules, and policies.
- National Association of Social Work’s Code of Ethics.
- Community resources.
- Ability to:
- Relate to individuals and families of varied ethnic and cultural backgrounds, ages and economic circumstances.
- Multitask while being proactive and adhering to schedules and deadlines.
- Communicate with clarity and accuracy; be accountable and responsible.
- Maintain self-management in controlling emotions and behavior; ability to use tact and diplomacy.
