Role Summary
The Care Manager - HCBS is responsible for coordinating person-centered Plans of Care and Service Plans for program participants. In this role, you will support individuals in selecting service providers, ensure timely initiation of authorized services, and monitor ongoing service delivery. You will maintain regular contact with participants and caregivers, identify changes in health or functional status, and collaborate with interdisciplinary team members to ensure coordinated, high-quality care and support.
Key Responsibilities
- Conduct comprehensive initial assessments and complete 60-day follow-up assessments after enrollment.
- Perform annual reassessments and additional assessments when significant changes in condition or needs occur.
- Develop individualized, person-centered Plans of Care based on participant needs and preferences.
- Coordinate monthly client visits and ongoing service delivery activities.
- Collaborate with participants, caregivers, families, healthcare professionals, and community providers to address identified needs.
- Support transitions of care, including coordination with hospitals, providers, and interdisciplinary team members.
- Maintain accurate and timely documentation of assessments, care coordination activities, and service plans within electronic systems.
- Participate in interdisciplinary team meetings, audits, and quality improvement initiatives.
- Educate participants and families regarding community resources, supports, and available program services.
- Ensure compliance with all regulatory, organizational, and program requirements.
Required Qualifications
- Associate degree (or higher) in behavioral/social sciences or related field OR at least 1 year of experience in health/human services or Current Licensed practical nurse in the state of Georgia.
- Minimum 2 years of case management experience (assessment, planning, implementation, monitoring, evaluation)
- Experience creating service plans and making recommendations for treatment or services
- Strong skills in relationship building, problem solving, and person-centered planning
- Proficiency with Microsoft Office and electronic documentation systems
- Valid driver’s license, automobile insurance, and reliable transportation for frequent travel
