Summary of Position
The Clinical Intake Specialist (LPN), under the supervision of a Registered Nurse (RN), serves as the primary point of contact for individuals with complex care needs. This role includes outreach, scheduling, conducting in-person visits, performing needs assessments, and coordinating care. The position offers a pay range of $22-$24 per hour. Responsibilities also include options counseling, care plan development, risk identification, collaboration with case managers, and educating participants and families.
Essential Functions
- Conduct initial face-to-face visits with referred individuals.
- Perform complex needs assessments.
- Complete quarterly, annual, and change-of-condition visits.
- Provide options counseling to clients and families.
- Develop individualized Plans of Care.
- Advise on cost-effective service planning.
- Maintain accurate documentation.
- Collaborate with healthcare professionals and service providers.
- Ensure compliance with laws and policies.
- Participate in IDT meetings.
- Report critical incidents and safety concerns.
- Triage and refer participants appropriately.
Position Requirements
- Associate degree in nursing or LPN diploma.
- Valid LPN license.
- Minimum 3 years of nursing experience.
- Experience in community or case management preferred.
- Knowledge of Medicaid/Medicare preferred.

Travel/Work Environment
Frequent travel required. Must have valid driver's license and insurance. Work occurs in homes, community settings, and office environments.
