Position Summary
The Manager, Complex Health is responsible for leading and overseeing the Complex Health Care Management team, ensuring the delivery of high-quality services in alignment with program standards and organizational policies. This role includes managing critical incident reviews, maintaining compliance with Columbus policies and procedures, and safeguarding the integrity of the Care Management Program.
Essential Functions
Care Management Oversight
- Lead and supervise person-centered care planning and cost-effective service coordination.
- Ensure all care activities comply with program standards and regulatory requirements.
- Demonstrate comprehensive knowledge of Community Care Services Program (CCSP) requirements, including eligibility criteria, covered services, consumer-directed care options, and Source Waiver policies and procedures.
- Manage consumer-directed care and monitor expenditures for home and community-based services.
- Provide guidance and oversight to staff regarding CCSP and Source Waiver requirements to support compliance and appropriate service delivery.
Compliance and Quality Assurance
- Conduct thorough reviews of critical incidents, identify root causes, and implement corrective actions.
- Maintain expert knowledge of Community Care Services Program (CCSP) and Source Waiver policies, regulations, eligibility requirements, and service delivery standards.
- Ensure timely and accurate incident reporting in alignment with organizational and regulatory policies.
- Develop and execute strategies to prevent incident recurrence and enhance patient safety.
- Participate in audits and prepare documentation to demonstrate compliance.
- Monitor adherence to policies, procedures, and standards of practice.
Team Leadership and Development
- Provide leadership and supervision to Care Management staff.
- Coordinate daily operations including staffing, scheduling, and resource allocation.
- Support professional development through coaching, training, and performance evaluations.
- Facilitate onboarding and ongoing education to promote continuous skill growth.
- Foster a collaborative and supportive team environment that encourages open communication and engagement.
Program Improvement and Reporting
- Lead quality improvement initiatives to enhance care management processes and patient outcomes.
- Use data and feedback to identify improvement opportunities and implement evidence-based practices.
- Monitor key performance indicators and report team performance to senior leadership.
- Facilitate regular team meetings to review cases, share best practices, and address challenges.
- Collaborate with other departments and healthcare providers to ensure seamless service delivery.
Position Requirements
- Associate’s or bachelor’s degree in nursing from an accredited institution.
- Current and valid Registered Nurse (RN) license required.
- Minimum 5 years of nursing experience.
- 1-2 years of experience in a leadership or management role required.
- Strong understanding of care management principles and practices.
- Regular travel required.
- Ability to work evenings and weekends as needed.
