About the Role:
As a Social Worker/Care Companion, you will work in collaboration with our experienced clinical team to provide comprehensive care to residents in skilled nursing facilities. Your focus will be on providing emotional support, brief therapeutic interventions, and mental health assessments to identify and address the emotional, cognitive, and psychosocial needs of our residents. You will work closely with psychiatric professionals to ensure comprehensive care and participate in monthly meetings with the psychiatric NP.
Key Responsibilities:
Visit designated facilities in accordance with the schedule set with your supervisor
Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
Document visit notes and maintain detailed records of interactions and interventions after each visit
Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP
Qualifications:
Associates, Bachelor's, or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field
Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
Strong communication, documentation, and relationship-building skills
1 year of experience

Flat salary: $22,000 - $24,000 annually
Location: 520 Glenburn Ave, Cambridge, MD 21613
Part-time schedule: approximately 15 hours per week during Monday-Friday, within regular 8-5 hours
