Journey of Care is a care coordination company providing the Remote Supports benefit through Colorado Medicaid Long-Term Care services. Our mission is to help high-risk individuals maintain independence, remain safely in their homes, and stay connected to ongoing support at no cost to the member.
We combine compassionate human connection with technology-enabled support to reduce crises, improve quality of life, and ensure members never feel alone. Through regular outreach, care coordination, safety monitoring, and collaboration with providers and caregivers, we act as an extension of the member’s support system.
We are also expanding services to support members and families across additional states and care programs.
Position Overview
The Care Coordinator / Remote Support Specialist serves as a primary point of contact for Journey of Care members, providing scheduled outreach, ongoing emotional support, care coordination, and proactive risk identification.
This role combines relationship-based care with remote support technology and resource coordination to help members remain safe, independent, and connected in their communities.
Team members assist individuals with:
- navigating healthcare systems,
- coordinating appointments and services,
- identifying unmet needs,
- accessing community resources,
- and responding to emotional, behavioral, or environmental concerns before they escalate.
This position is ideal for someone who is compassionate, organized, emotionally intelligent, and comfortable building meaningful relationships over the phone.
Key Responsibilities: Member Outreach & Emotional Support
- Conduct scheduled outreach calls to assess wellbeing and provide ongoing support.
- Build trusting relationships through compassionate, person-centered engagement.
- Provide emotional support and meaningful connection to reduce isolation and crisis risk.
- Respond to inbound support calls and member concerns.
Care Coordination
- Assist members in navigating healthcare and community-based services.
- Coordinate with caregivers, providers, pharmacies, case managers, and family members as appropriate.
- Help members access medical appointments, DME, safety equipment, transportation, and community resources.
- Support communication with providers and assist members in advocating for their needs.
Safety Monitoring & Risk Prevention
- Monitor remote safety technologies, sensors, and alerts.
- Recognize signs of physical decline, behavioral health changes, social isolation, financial stress, or environmental safety concerns.
- Follow escalation protocols and complete incident reporting when needed.
Documentation & Compliance
- Complete accurate and timely documentation of all member interactions and interventions.
- Maintain HIPAA compliance and confidentiality standards.
- Follow Medicaid and organizational documentation requirements.
Required Qualifications
- Minimum 1 year of experience in healthcare, care coordination, behavioral health, or medical office settings.
- Strong communication, organizational, and documentation skills.
- Compassionate and patient-centered approach.
- Ability to manage emotionally complex conversations professionally.
- Comfortable using remote technology, tablets, and online systems.
- Ability to work independently in a remote setting.
Preferred Qualifications
- Experience working with Medicaid or Medicare populations.
- Experience supporting older adults or individuals with disabilities.
- Previous care coordination, case management, behavioral health, or caregiving experience.
- Student pursuing nursing, social work, counseling, or another healthcare-related field.
- Bilingual or multilingual abilities.
Work Environment & Schedule
- Fully remote position, with some on-site team events
- Flexible scheduling based on member needs
- Some evening or weekend availability may be required
Pay: $25.01 - $32.12 per hour
