JOB SUMMARY
The Social Worker is responsible for completing Social Services related tasks. Job responsibilities may include but are not limited to:
- Perform all Social Services tasks and duties for the facility.
- Build and maintain good relationships with residents and families.
- Be a resident advocate where the resident is the center of all we do.
- Ensure excellent & timely communication with residents, families, staff, and referral sources.
- Create positive resident outcomes.
- Respect others by treating them the way we want to be treated.
- Demonstrate kindness, compassion, honesty, and integrity at all times.
- Help create a culture of superior customer service.

REQUIREMENTS
- Successful criminal background check
- Bachelors Degree in social work or related field
- Excellent communication and customer service skills
- Long-term care facility experience preferred
- Social Services licensure, where applicable
