Job Description

DEFINITION AND DUTIES:

Under general supervision of the Social Services Director, assist individuals and families with accessing various social services, including but not limited to healthcare, housing, food and financial assistance. Work collaboratively within a multi-disciplinary approach; and assist individuals and family members with navigating social and community services Provide staff supervision and support. Coordinate the Johnson County Livable Community for Successful Aging Policy Board.

Knowledge of:
  • Community resources available to meet the needs of low income and elderly residents
  • Federal, State and local resources including; Medicaid, Medicare and other entitlement programs
  • The aging process and specific characteristics and needs of the elderly and the continuum of residential care options for aging in place
  • Principles of supervision, to include performance appraisal methods, approaches to discipline and training techniques.
  • Principles of program planning, quality improvement, performance management and assurance.
  • Community organizing and collaboration strategies
  • Cultural competence in community organizations and community-based services
  • Basic office practices and procedures including record keeping and filing systems
  • Techniques of interviewing, including accurately collecting, evaluating, verifying and recording information
  • English language to include grammar, spelling, sentence structure, vocabulary and proofreading skills

  • Have clarity of speech and hearing which permits effective communication
  • Have sufficient vision which permits moderate production and review of a wide variety of materials in both electronic and hardcopy forms
  • Have sufficient manual dexterity to make handwritten notations and which permits moderate use of a keyboard and mouse
  • Have sufficient personal mobility to complete work and attend meetings at various county facilities and other locations
  • Stand, sit, squat, kneel, bend, twist, and reach as needed
  • Lift and/or move up to 25 pounds as needed
  • Occasionally work flexible schedule and overtime as needed


MINIMUM EDUCATION, TRAINING AND EXPERIENCE:

Master's degree in social work or related field including but not limited to psychology or sociology AND one (1) year supervisory experience; OR Bachelor's degree in human services related field including but not limited to social work, psychology, or sociology AND three (3) years of experience in human services field including one (1) year in a supervisory capacity; OR any equivalent combination of education and experience which provides the required knowledge, skills and abilities. . Experience working with low-income individuals preferred.

Special requirements: Must pass background investigation. Must have current Child and Dependent Adult Abuse certification, or complete certification within two (2) months of employment. Valid driver's license and insurable under County liability coverage. May require use of personal vehicle or equivalent transportation for official business.

SPECIFIC DUTIES: to be performed satisfactorily with or without reasonable accommodation

The following duties are normal for this position. These are not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

  • Supervise staff, including recruitment, training, performance evaluation and disciplinary actions in consultation with Director as appropriate.
  • Develop and coordinate plans that identify and meet the social service needs of County residents and reduce barriers to accessing services.
  • Utilize a strength-based and trauma-informed care approach to assess multi-disciplinary social needs and provide advocacy, support and short-term coordination of services for individuals and families.
  • Provide information, referrals, and aid in accessing supportive services. Provide follow up to ensure consumer accessed needed services.
  • Improve wellbeing for individuals and families by assisting with reducing barriers to affordable housing, education, transportation, food and financial assistance, transportation, aging services, childcare, mental health and support services.
  • Guide clients through complex systems, such as healthcare, mental health services, community resources and government benefits.
  • Provide family caregiver counseling, support, and resource and referral services.
  • Work collaboratively with area service organizations to effectively coordinate services for individuals and families.
  • Assist individuals with the completion of applications for various assistance programs.
  • Coordinate Johnson County Livable Community for Successful Aging Policy Board, providing new board member orientation preparation and posting of agendas and completion of minutes.
  • Coordinate student, volunteer, intern and practicum placement within the department.
  • Conduct community outreach and speak to community groups regarding social services.
  • Participate in community-based coalitions and committees.
  • Assist with developing and maintaining various resource materials and directories including the online Johnson County Livable Community Provider Directory.
  • Provide home visits and community-based services and outreach; facilitate trainings regarding senior issues and anti-poverty initiatives.
  • Maintain records and documentation for individual client files.
  • Perform general clerical duties such as filing, photocopying and completing internal forms.
  • Attend work-related meetings, trainings, and conferences as required.
  • Provide back-up for co-workers.
  • As a mandatory reporter, maintain approved training on child and dependent adult abuse reporting requirement.
  • Perform all work duties and activities in accordance with County policies, , procedures, and safety practices.
  • Attend work regularly at the designated place and time.
