Full-Time Palliative Care CoordinatorIn-Office Position | Pasadena, CAHelp coordinate compassionate care that truly makes a difference. We are seeking an organized, compassionate, and detail-oriented Palliative Care Coordinator to join our growing team. This position plays a key role in coordinating patient care, scheduling services, processing referrals, obtaining authorizations, and supporting our interdisciplinary care team. Schedule: Monday - Friday | 9:00 AM - 6:00 PM $21.00 - $25.00/hour (DOE) Spanish speaking preferred What We're Looking ForStrong computer and customer service skills Excellent communication and organizational abilities Hospice, home health, or palliative care experience preferred Ability to multitask in a fast-paced healthcare environment Professional, compassionate, and team-oriented attitude Valid driver's license and reliable transportation Key ResponsibilitiesCoordinate palliative care referrals and admissions Obtain insurance authorizations and re-authorizations Schedule patient visits and coordinate care services Communicate with physicians, nurses, social workers, and care teams Review documentation for accuracy and timely submission Support patient and family needs while ensuring quality care delivery Maintain accurate patient records and reports Why Join Our Team?We believe compassionate care starts with compassionate staff.
