Adult Day Health Care Social Worker / Care Coordinator

Altamedix Adult Day Health Care is seeking a compassionate and motivated Social Services professional to join our interdisciplinary team serving seniors and medically complex adults in a supportive community-based healthcare setting.

This position offers meaningful long-term participant relationships, stable daytime hours, and the opportunity to make a direct impact on the quality of life of vulnerable older adults and their families.

Position Highlights

Qualifications

  • Bachelor’s or Master’s degree in one of the following fields: Social Work, Psychology, Gerontology, Counseling
  • Strong interpersonal and communication skills
  • Ability to work collaboratively within an interdisciplinary healthcare team
  • Experience working with seniors, disabled adults, or medically complex populations preferred
  • Bilingual candidates (Russian, Ukrainian, Spanish, Arabic, Farsi, or other languages) are strongly encouraged to apply

Essential Duties and Responsibilities

  • Conduct psychosocial assessments for program participants
  • Develop and maintain individualized plans of care in collaboration with the interdisciplinary team
  • Provide individual and group counseling interventions in accordance with participant care plans
  • Coordinate referrals to community resources and support services
  • Serve as a liaison between participants, families, caregivers, and outside agencies
  • Participate in interdisciplinary care conferences and team meetings
  • Maintain accurate, timely, and compliant documentation and social service records
  • Support participant advocacy, emotional well-being, independence, and quality of life
