Under new management, we're growing our team and looking for an experienced Client Care Manager (RN) who takes pride in their work and wants a stable, supportive home.
Overall Objective/Purpose Statement: Under the supervision of the Branch Manager, this position — which must be held by a Licensed Registered Nurse (RN) in good standing with the California Board of Registered Nursing — is responsible for planning and managing all aspects of client care including intake, start of care and ongoing service needs. Responsible for creating and maintaining accurate care plans for all clients and ensuring that caregivers are in compliance with care plan directives. Provides initial client assessment, and periodic reassessment to ensure appropriate level of care is assigned.
Location: Predominantly located at 259 S Randolph Ave #180K, Brea, CA 92821. With occasional visits to client homes and medical offices as needed.
Part-Time: 20 Hours Per Week
Essential Job Duties:
Assesses client needs and develops a care plan for each assigned client by identifying appropriate level of care, setting schedules and routines; coordinating services provided.
Supervises and instructs caregiver on how to execute the care plan, ensuring comprehension of unique care needs of their assigned clients.
Maintains an active, unrestricted California Registered Nurse (RN) license in good standing with the California Board of Registered Nursing; immediately notifies the Branch Manager of any change in licensure status or disciplinary action that may affect eligibility to practice.
Conducts required supervisory visits for all RCOC-contracted consumers in accordance with Regional Center of Orange County vendor contract requirements, documenting observations, care plan compliance, and any change in condition at each visit.
Submits all RCOC-required documentation — including supervisory visit notes, updated care plans, and incident reports — within required timeframes, maintaining consumer records in accordance with Title 17 regulations and RCOC vendor standards.
Performs assessments according to company protocol on a weekly or as-needed basis for each assigned client.
Reviews client’s care plan for accuracy and safety, updating with new information or change of condition, communicating changes to staff.
Completes “rounds” to client homes on a regular basis to observe, instruct, or assist caregiver, assess clients, and ensure all necessary equipment and supplies are available.
Collects and reviews case documentation to ensure that all records are completed accurately.
Reviews care tasks assigned to each caregiving shift through scheduling software to ensure tasks are completed as assigned.
Evaluates each caregivers compliance with care plan for their assigned clients and identifies individual, or groups of caregivers to receive additional training as needed.
Attends all scheduled doctor visits for assigned clients; visits clients admitted to emergency department, hospital, or rehab facility, assisting family members in coordinating discharge and care.
Ensures medications are arranged by responsible party, communicates changes in medication schedule to caregivers.
Performs new client intake, meeting with client and responsible parties to determine care needs. reviewing records and performing intake interview; conducting orientations, explaining services offered to the client and family members.
Completes company intake forms. Collects all relevant client data including demographics, medical conditions, family and responsible party information, home and safety information, and preferences, documenting information in systems according to company protocol.
Responsible for maintaining the accuracy of digital records by inputting and updating client demographic and medical information, home safety assessments, tracking hospital admissions and release date, assigning tasks to the appropriate shift, producing ADL reports as needed.
Cooperates with all members of the Client Care Team by communicating new information effectively; reviewing ongoing cases; implementing recommended changes.
Attends team meetings and all staff meetings providing status updates and communicating unresolved issues.
Records information on each event and prepares detailed communication on all activities occurring during the shift; shares information with the Client Care and Administrative Teams according to company protocol.
Upholds company policies by responding to questions, reinforcing procedures, resolving work-related problems. Observed deficiencies or breaches of policy must be documented and corrected or referred to HR for follow-up.
Participates in new hire 90-day retention plan by contacting employees on assigned dates and ensuring steps are completed
Responsible for inputting and completing Axiscare shifts for all office time, client visits, and on-call shifts.
Assists in covering the on-call Client Care Manager phone line.
Protects organization's value by keeping information confidential.
Takes on additional special projects as directed by the Branch Manager
Professional Requirements:
Minimum two years experience in a related field such as nursing.
Must have a current certification as a Registered Nurse in the state of California.
Must have a current Basic Life Support Certification.
Supervisory or team leading experience in long-term care preferred.

Relationships: Reports to and is supervised by the Branch Manager. This position participates in the supervision and management of caregivers.
