Bella-Nacole Mental Health Services and Diamond Peak Health Care work hand-in-hand with Idaho Medicaid and the Department of Behavioral Health to provide stable housing and life-changing mental health services for Idaho’s most chronically mentally ill.
At the HART (Housing for Adult Residential Treatment) Program, we joke, we laugh, we tease, and we genuinely enjoy working together. About the HART Program
The HART program is a specialized, intensive program designed for Idaho’s Severe and Persistent Mental Illness (SPMI) population (members with diagnoses like schizophrenia, bipolar, PTSD, etc.). By combining therapy, CBRS, case management, primary care, and medication management under one roof, we provide an unprecedented level of coordinated care. Our goal is simple but profound: aid members in achieving stability, maintaining autonomy, and staying out of hospitals and jail.
What You’ll Do (The Role)
As a CPSS, you know firsthand what it’s like to navigate a mental health journey. You will use your lived experience to offer active listening, validation, and empathy, helping reduce isolation and stigma. You'll address the needs of the whole person—mind, body, and spirit—by focusing on four core elements:Support, Hope, Opportunity, and Tools.
Key Responsibilities:
- Be a Champion of Hope: Share your recovery experience and knowledge to encourage self-determination and self-confidence.
- Goal Setting: Help clients set health and recovery goals, develop action plans, and build daily management skills.
- Community & Navigation: Connect clients with professional and community resources, and assist them in navigating the service system independently.
- Holistic Support: Encourage exercise, good nutrition, and problem-solving while providing emotional support and feedback.
- Health Appointments: Help clients prepare for healthcare visits and practice asking questions.
What We’re Looking For (Qualifications)
Must-Have: You must hold a current Idaho Peer Support Certification and have personal lived experience with mental illness and recovery. You must be healthy enough to provide support to others struggling.
- Education: High School Diploma or GED.
- Transport: Must have a reliable personal vehicle to use for community-based services, a valid driver's license, and up-to-date auto insurance.
- Skills: Strong documentation skills, a sharp eye for date-sensitive materials, and absolute compliance with HIPAA/confidentiality laws.
- The Vibe: Reliable, ethical, open-minded, and possessing excellent professional boundaries—while still being able to laugh and play with a tight-knit team.
