Job Description

We are seeking a dedicated and experienced Medical Social Worker (MSW) to join our team on a PRN basis. This role is ideal for a professional seeking a flexible schedule while making a meaningful impact on patient lives. You will work closely with our clinical team to assess social and emotional factors related to patient illness and provide necessary interventions.

Key Responsibilities

• Conduct social and emotional assessments of patients in their home environment.
• Assist patients and families with community resource planning, long-term care arrangements, and financial assistance applications.
• Provide short-term crisis intervention and counseling related to the patient’s medical condition.
• Collaborate with the multidisciplinary team (RNs, PTs, OTs) to develop and update the plan of care.
• Complete and submit clinical documentation (via Kinnser/WellSky) within our 24–48 hour compliance window.

Qualifications

• Master’s Degree in Social Work (MSW) from a school accredited by the Council on Social Work Education.
• Current state license to practice as a Social Worker.
• Minimum of 1 year of experience in a healthcare setting (Home Health or Hospice experience preferred).
• Strong knowledge of local community resources and Medicare/Medicaid guidelines.
• Excellent communication and organizational skills.
• Reliable transportation and valid driver’s license.

Job Types: Contract, PRN, Per diem, Part-time

Pay: $75.00 per hour

Work Location: On the road
