Compensation:
$87,658.16 – $131,487.33 annually (Full Time)

Position Summary
The Social Work Care Manager is responsible for completing comprehensive patient assessments and coordinating discharge planning and transitional care services for hospitalized patients. This role works collaboratively with interdisciplinary healthcare teams to identify high-risk patients, reduce readmissions, improve patient outcomes, and ensure safe and appropriate discharge planning.
Key Responsibilities
- Complete comprehensive psychosocial and discharge planning assessments upon admission
- Identify high-risk patient populations requiring supportive services
- Coordinate multidisciplinary care planning throughout hospitalization
- Facilitate discharge planning and transitional care services
- Provide patient and caregiver education regarding aftercare plans
- Coordinate post-acute services and community resources
- Collaborate with physicians, nursing staff, payers, and interdisciplinary teams
- Assist with readmission prevention and length-of-stay reduction initiatives
- Document assessments, interventions, and discharge planning activities
- Participate in quality improvement and performance initiatives
Education & Experience Requirements
- Master of Social Work (MSW) from an accredited School of Social Work required
- LMSW or LCSW licensure through New York State required
- Minimum of 2 years of acute care hospital or related experience preferred
- NYS SCREEN Certification required within 3 months of hire

Pay: $87,658.00 - $131,487.00 per year
