LPN/RN – Care Coordinator

Position Summary:
The Licensed Practical Nurse (LPN)/Registered Nurse (RN) Care Coordinator supports patient-provider communication by managing telephone encounters, triage patient calls, relaying results, and providing education and care coordination. This role offers flexibility to work remotely and onsite, improving patient access and care continuity. This position follows a Monday through Friday schedule and is hybrid, with the ability to work remotely as approved.
Key Responsibilities:
- Answers and triage of patient calls based on clinical urgency.
- Respond to all nurse triage events for walk in patients.
- Document patient symptoms and interactions clearly in eClinicalWorks.
- Coordinate provider responses and relay instructions to patients.
- Ensure timely return of all patient communications, including voicemails, web encounters, and patient portal messages, per clinic policy.
- Coordinate in-person encounters as needed to support high-quality, patient-centered care.
- Create, manage, and close telephone encounters promptly.
- Schedule patient appointments as needed.
- Relay lab and diagnostic results to patients as directed by providers in a timely manner.
- Assist with prescription refills, prior authorizations, and referrals.
- Educate patients about managing acute and chronic conditions.
- Support follow-up scheduling and care gap closure; assist with chronic care management
- Always Protect patient confidentiality (HIPAA compliance).
- Participate in clinical quality improvement and team meetings.
Qualifications:
- Successfully completed an accredited nursing degree program and maintains an active, unrestricted nursing license in good standing.
- CPR certification.
- Experience with telephone triage is preferred.
- Proficiency in Electronic Health Records (eClinicalWorks preferred).
- Reliable internet connection for remote work.
- Preferred:
- Experience working in an FQHC, outpatient clinic, or family medicine practice.
- Formal training or certification in Health Information Management (HIM) is a plus.

OCCUPATIONAL HEALTH AND SAFETY
Approximately 75% of work may be performed in a typical office setting, with the remaining time eligible for remote work depending on operational needs. The role requires moderate physical activity during on-site work, including lifting to 25 lbs. and standing or walking for over four hours per day.
