Duties

  • Conduct comprehensive patient assessments to evaluate emotional, social, and behavioral health needs within hospice care settings.
  • Develop individualized care plans in collaboration with clients and their support systems.
  • Provide counseling and crisis intervention services to individuals and families.
  • Coordinate with external agencies and service providers to ensure clients receive necessary support.
  • Advocate for patients’ rights and access to resources
  • Monitor patients' progress and adjust care plans as needed.
  • Maintain accurate and up-to-date documentation of client interactions and services provided.
  • Educates patients and families and assist in preparation of advanced of advanced directives.
  • Collaborate with interdisciplinary teams—including medical staff, nurses, therapists, and caregivers—to develop personalized care plans aligned with hospice goals.
  • Assist with case management duties such as coordinating community resources, facilitating family meetings, and managing medical documentation in compliance with HIPAA regulations.
  • Engage in clinical research activities related to hospice care practices and patient outcomes to promote continuous quality improvement.
  • Maintain accurate medical records using electronic medical record (EMR) systems, ensuring documentation reflects diagnostic evaluations, treatment plans, and progress notes.
  • Requirements
  • Degree in Social Work (LMSW/LCSW)
  • Proven experience in hospice care, palliative services, or related healthcare environments such as hospitals, nursing homes, or assisted living facilities.
  • Skilled in crisis management techniques and crisis intervention strategies within home & community care contexts.
  • Proficiency with EMR systems for data collection and medical records management; familiarity with inpatient settings is a plus.
  • Excellent communication skills for intake assessments, diagnostic evaluations, and delivering psychotherapy across individual or group formats.
  • Ability to work independently while collaborating effectively within multidisciplinary teams to deliver compassionate patient-centered care.
