Comfort Keepers is looking for a service-driven and organized Care Coordination Specialist to join our team in Fayetteville and surrounding communities.

This role focuses on coordinating care services, supporting families and caregivers, and ensuring smooth communication and high-quality service delivery throughout the care process.

Service Areas May Include:
Fayetteville, Chambersburg, Shippensburg, Carlisle, Mechanicsburg, Camp Hill, and nearby surrounding communities.

Pay Rate: $30,000 – $45,000 per year

Role Overview

• Act as a central contact for clients, families, and caregiving staff
• Coordinate caregiver schedules and daily care assignments
• Assist with client onboarding and service setup
• Maintain communication between families, caregivers, and office staff
• Respond to scheduling changes and service concerns promptly
• Maintain accurate records, reports, and documentation
• Participate in community outreach and engagement activities
• Support process improvements and coordination efforts
• Collaborate with the team to maintain exceptional care standards
