What You’ll Do

  • Conduct phone and walk-in screenings for prospective patients
  • Coordinate assessments, admissions, and insurance verification
  • Complete intake paperwork accurately and in compliance with policies
  • Collaborate with physicians, nurses, and clinical staff on patient care plans
  • Monitor patient progress and provide daily follow-up support
  • Develop personalized discharge and aftercare plans with patients
  • Report daily census updates to leadership
  • Conduct outreach calls to referral sources and community agencies
  • Complete biopsychosocial assessments
  • Support additional patient care coordination needs as assigned

What We’re Looking For

  • Bachelor’s degree in Human Services or related field
  • 2+ years of social services, behavioral health, or clinical experience
  • Knowledge of substance use disorders and recovery support
  • Detox, MAT, case management, or substance abuse counseling experience preferred
  • Strong communication and relationship-building skills
  • Ability to thrive in a fast-paced, patient-centered environment
  • Hospital or acute care experience preferred
  • Ability to pass background check and drug screening
