QUALIFICATIONS:

  • BA degree in social work or related health or human service field.
  • Prior experience with client engagement, customer service, or social service interventions preferred.
  • Case management experience preferred.
  • Bilingual (English/Spanish) preferred.
  • Demonstrated strong interpersonal and customer service skills, with the ability to work effectively and respectfully with a multicultural client population, engage in a culturally sensitive and nonjudgmental manner that supports positive outcomes, and work collaboratively as part of a team while maintaining appropriate professional boundaries.
  • Excellent communication skills, both verbal and written, and ability to utilize telehealth platforms.
  • Current unrestricted CA Driver’s License and proof of auto insurance.
  • Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment.


RESPONSIBILITIES/ESSENTIAL FUNCTIONS:

  • Respond to intake calls in a timely and professional manner to assess needs and provide information and referrals.
  • Provide connections to internal JFSLA programs and community referrals as indicated.
  • Provide direct front-facing client engagement and customer service at JFSLA SOVA sites and community-based pop-up locations.
  • Provide short-term targeted case management services, including ongoing follow-up and development of individualized case management plans and goals in collaboration with clients.
  • Support clients in identifying available government and community resources and assist, as needed, with advocacy and navigation
  • support to connect to those resources.
  • Work collaboratively with SOVA staff on outreach efforts, client registration, and client engagement to help connect community members to additional targeted case management services.
  • Manage multiple tasks and prioritize effectively.
  • Provide timely documentation in accordance with funding requirements and electronic health record standards.
  • Participate in weekly individual supervisory meetings.
  • Other related duties as assigned.
