Essential Job Functions and Responsibilities
1. Meet with new program participants to complete initial assessments.
2. Maintain scheduled appointments with program participants to provide intensive case management.
3. Maintain regular contact with program participants to provide ongoing supportive services.
4. Advocate on behalf of participants and connect them to supportive services.
5. Assist participants in obtaining public benefits.
6. Maintain scheduled home visits with existing participants.
7. Keep and maintain accurate and up to date participant files.
8. Input and update data in the Homeless Management Information System.
9. Participate in on site and off site trainings and meetings as needed.
10. Develop and maintain professional relationships with landlords.
11. Other duties as specified
Preferred Qualifications
1. Minimum of a Bachelor’s degree in social work, psychology, or other related social services field.
2. Possess a valid Driver’s License and reliable transportation for local travel.
3. Experience in working with individuals that use substances and have varying mental health diagnosis.
4. Experience in working with homeless and/or low-income population.
5. A minimum of one-year of experience providing case management.
6. Experience in working with the Suburban Cook County Coordinated Entry System.
7. Experience using the Homeless Management Information System of Suburban Cook County.
Pay: From $20.00 per hour
