JOB SUMMARY:

The Madrone Care Network (MCN) Social Worker (LCSW) provides direct palliative care social work services as part of the transdisciplinary team. This position reports to the Social Work Manager and actively participates in the delivery of specialty palliative care.

This position is based in the Eureka area, serving surrounding communities. This position may be filled by an MSW, ASW or LCSW candidate. Pay range is between $43 and $50 per hour based on education/licensure type.

ESSENTIAL JOB FUNCTIONS:

Provides direct medical social work to patients and their families through in-person visits, telephone, and video-conference mediums

Assesses patient and family psycho-social needs and develops an interdisciplinary plan of care

Provides care management, crisis intervention, and short-term psycho-therapeutic interventions to patients and families as defined by license and training

Collaborates with the interdisciplinary team to assess and refer patient medical and spiritual care needs to clinical and chaplaincy team members as appropriate

Assists patients with the completion and understanding of advance care planning documents

Acts in accordance with LCSW licensure and California mandated reporter legal requirements

Documents in EMR, care coordination tracking systems, and additional applications as required for patient care billing and contractual requirements

Utilizing appropriate data-collection and patient screening tools as assigned (including but not limited to PCQC and PHQ-9)

Participates in the MCN interdisciplinary team meeting and community multidisciplinary team meetings

While work is conducted in person in multiple work locations, this position requires access to a remote office/home office location.

PHYSICAL REQUIREMENTS:

Work locations will vary and frequent driving is required. Work may be conducted from MCN’s office, a patient’s residence, public community locations, residential care facilities, physician offices, hospital locations or other locations where a patient may reside or receive care. Requires frequent use of computer platforms and electronic medical records.

Ability to sit for extended periods, as well as frequent walking, standing, bending, and reaching.

Ability to operate office equipment, type, handle paperwork, perform data entry tasks, complete medical chart updates, deliver supplies to patients and drive.

Ability to hear and communicate effectively verbally and in writing with patients, caregivers, team members, and community partners, with or without reasonable accommodation.

Ability to read, review, and analyze written documents and electronic records and drive to meet and deliver to patients.

Ability to lift, carry, or move items weighing up to 25lbs.

The work environment includes patients’ homes and community locations and may involve exposure to bodily fluids, communicable illnesses, pets, smoke, environmental allergens, uneven surfaces, and varying home conditions. Adherence to established safety, infection control, and PPE guidelines is required. May involve exposure to unknown or potentially hazardous household chemicals, cleaning agents, or substances not under the organization’s control. Employees are expected to follow Cal/OSHA safety standards, hazard awareness training, and safety protocols.

QUALIFICATIONS:

The MCN LCSW must posses education/licensure at one of the following levels:

  • LCSW - Possess a master's degree in social work from a CSWE-accredited program, accrued at least 3,200 hours of supervised work experience, and passed the Association of Social Work Boards (ASWB) clinical-level exam. Have current licensure as a LCSW from the California Board of Behavioral Sciences Experience in medical social work.Maintain CE requirements for the California Board of Behavioral Sciences
  • MSW -LCSW - Possess a master's degree in social work from a CSWE-accredited program.
  • ASW - LCSW - Possess a master's degree in social work from a CSWE-accredited program. Have a current licensure as an ASW from the Have current licensure as a LCSW from the California Board of Behavioral Sciences Experience in medical social work.Maintain CE requirements for the California Board of Behavioral Sciences.

Have or be eligible for membership in good standing on the medical staffs of hospitals within MCN’s service area.

Be proficient in Mac software suite and Google calendar/scheduling applications

Have and maintain a valid Driver’s License, maintain automobile insurance coverage and have access to transportation between primary work location and patient locations within the community.

Pay: $43.00 - $50.00 per hour
