A respected nonprofit healthcare and reentry-focused organization is seeking a compassionate and clinically skilled LMSW or LMHC to join a growing behavioral health team supporting individuals within New York City correctional facilities.

This is a meaningful opportunity for a clinician who believes in trauma-informed care, rehabilitation, and helping underserved populations access behavioral health and substance use support services. The role offers a collaborative environment, mission-driven leadership, and the ability to make a direct impact on vulnerable communities.

Key Responsibilities:

  • Conduct biopsychosocial assessments and evidence-based screenings related to mental health and substance use disorders.
  • Prepare detailed clinical documentation and recommendations to support alternative treatment programming, specialized care placement, or community-based services.
  • Collaborate with multidisciplinary clinical teams, peer specialists, and case management staff to coordinate individualized support plans.
  • Provide brief interventions, crisis support, safety planning, and risk assessments when clinically indicated.
  • Maintain timely and accurate case documentation within electronic health record (EHR) systems.
  • Assist with referrals to behavioral health, substance use treatment, care management, housing, and community support services.
  • Participate in case conferences, team meetings, supervision sessions, and ongoing professional development opportunities.
  • Track scheduled and completed assessments while managing changing priorities within a fast-paced clinical environment.
  • Support continuity of care and discharge planning efforts for individuals transitioning back into the community.

Qualifications:

  • Active LMSW or fully licensed LMHC required in New York State.
  • Minimum of 1 year of clinical experience in behavioral health, mental health, substance use treatment, crisis intervention, correctional health, or community-based services.
  • Experience working with justice-involved, trauma-affected, or underserved populations strongly preferred.
  • Familiarity with reentry challenges and barriers to accessing behavioral healthcare is highly valued.
  • Strong written documentation, organizational, and communication skills.
  • Ability to thrive in a collaborative, mission-focused, and highly dynamic environment.
