Tailored Care Management (TCM) Care Manager

Job Title: Licensed Tailored Care Management (TCM) Care Manager (Hybrid)

Job Overview

Walkers Group Home is hiring Licensed Care Managers and Qualified Professionals (QPs) to provide Tailored Care Management (TCM) services. In this role, you will coordinate whole-person care for individuals with complex needs, including people living with mental illness, substance use disorders, and/or intellectual and developmental disabilities. We’re looking for someone who is compassionate, organized, and committed to helping people reach greater stability and independence.

Key Responsibilities

• Complete comprehensive assessments of clients’ health, behavioral health, and social needs
• Create and update person-centered care plans with clients, families, and the care team
• Coordinate services across medical, behavioral health, and community providers
• Monitor progress and adjust care plans to support client goals and outcomes
• Educate and support clients and families to navigate services and resources
• Deliver culturally responsive, person-centered services that respect client preferences
• Partner with community resources to reduce barriers (housing, transportation, employment, etc.)
• Document services accurately and on time, following agency policies and all state/federal requirements
• Participate in team meetings and care coordination conferences
• Advocate for clients across healthcare, judicial, and social service systems as needed

Qualifications

• Bachelor’s or Master’s degree in social work, nursing, counseling, or a related field
• Must meet requirements as a Licensed Care Manager and/or Qualified Professional (QP)
• At least 2 years of experience in care management, case management, or a related role
• Strong understanding of care coordination and working with individuals with complex needs
• Excellent communication, organization, and problem-solving skills
• Experience working with diverse populations and a commitment to cultural humility
• Valid driver’s license and reliable transportation (some in-person visits required)
• Ability to complete NCI-B and CPR training
• Ability to lift up to 25 pounds and regularly walk, stand, or stoop
