Position Overview

Avatar Home Health & Hospice is seeking a dynamic and experienced Community Liaison to support growth and strengthen referral relationships within the Houston Medical Center and surrounding territories. This role is ideal for a relationship-driven professional with deep knowledge of home health and hospice services who thrives in a fast-paced, mission-focused environment.

As a Community Liaison, you will serve as a key ambassador for Avatar—educating referral sources, supporting smooth patient transitions, and ensuring our services are accessible to those who need them most.

Key Responsibilities

• Develop and maintain strong relationships with physicians, hospitals, case managers, and community partners
• Promote Avatar’s home health and hospice programs while upholding the highest standards of quality care
• Assess potential patients for eligibility and coordinate care transitions
• Serve as a liaison between referral sources, clinical teams, patients, and families
• Identify growth opportunities and contribute to strategic outreach initiatives
• Manage territory coverage throughout North Houston, TX, with office support based in Spring, TX
• Ensure compliance with all regulatory and ethical standards while maximizing resources effectively

Qualifications

• Experience: Required background in the home health and/or hospice industry
• Education: Graduation from an accredited college or university preferred
• Skills & Attributes:
• Strong interpersonal and communication skills
• Proven ability to build trust and long-term professional relationships
• Self-motivated, goal-oriented, and persistent
• Professional, personable, and confident in community-facing settings
• Ability to travel regularly within assigned territory
