Responsibilities:

• Assess the social and emotional needs of patients and develop personalized care plans to address these needs

• Provide counseling and therapy sessions to patients and their families to promote emotional well-being and coping skills

• Facilitate communication between patients, families, and healthcare providers to ensure seamless care coordination

Requirements:

• 2+ years' experience as Social Worker in Skilled Nursing Facility

• Strong communication and interpersonal skills to effectively engage with patients, families, and healthcare providers

• Ability to work in a fast-paced environment with multiple priorities
