Duration - 13 Weeks with possibility of extension
Shift -Day 5x8-Hour (08:00 - 16:30)

Position Summary:
Compassionate and dedicated Master Social Worker with experience providing psychosocial assessments, care coordination, crisis intervention, and patient advocacy in healthcare and community settings. Skilled in developing individualized care plans, connecting patients and families with community resources, and collaborating with multidisciplinary teams to support emotional, social, and behavioral well-being.

Job Duties:

• Conducted comprehensive psychosocial assessments to evaluate patients’ emotional, social, financial, and environmental needs.
• Developed and implemented individualized care plans in collaboration with patients, families, and interdisciplinary healthcare teams.
• Provided crisis intervention, emotional support, counseling, and patient advocacy services.

Requirements:

• Experience: 2 years
• Certifications: BLS, LCSW*, MSW
