Job Title: Care Coordinator

Department: Wraparound
Location: Eureka, CA
Status: One (1) Full-Time - Non-Exempt/Exempt
Hours: 40 hours per week
Salary: $72,072.00 Depending on Experience / Additional Differentials Must Qualify

Pay Differentials Offered (must qualify through testing or verification):

  • Language Translation - Bi-cultural Spanish: $1.00 per hour
  • Bimodal Bilingualism - ASL Sign Language: $1.00 per hour
  • Bimodal Multilingualism - ASL Sign Language: $2.00 per hour

Overview:

Responsible for coordinating and facilitating family-centered, multidisciplinary teams focused on assessing and addressing the individualized needs of children and their families. This includes identifying strengths and challenges, developing tailored service plans, and connecting families with appropriate community resources and support systems to promote stability, well-being, and long-term success.

Key Responsibilities:

  • Conducts initial meetings with families to introduce the wraparound team approach, securing all necessary consents for services and release of information.
  • Organizes and facilitates Child and Family Team (CFT) meetings to develop, review, and revise the Family Care Plan, ensuring a strength-based, family-driven process that promotes decision-making and consensus building.
  • Reviews and submits billing forms, reports, and related documentation prepared by support staff for accuracy, completeness, and compliance with agency policies.
  • Identifies and reports clinical concerns to program managers and supervisory staff in a timely manner
  • Performs comprehensive strengths, needs, and cultural assessments to gather contextual information critical to individualized care planning.
  • Coordinates and leads wraparound team meetings focused on developing care plans that integrate both professional services and natural supports to address the intensive, community-based treatment needs of children and their families.
  • Ensures collaborative development and ongoing revision of service coordination plans, including proactive safety and crisis intervention strategies.
  • Facilitates access to formal and informal support systems, fostering communication between families, natural support, community resources, and child-serving agencies.
  • Provides continuity of care by managing transitions across service levels and supporting older youth through the transition to adulthood.

Classifications and Qualifications:

  • Must have one of the following:
    • Bachelor of Social Work (BSW), Or Bachelor's degree (BS), Master's degree (MA) in social science related field; and at least two years related experience and/or training; or equivalent combination of education and experience and/or Licensed or Intern with BBS.
  • Specialized knowledge and understanding of at-risk youth involved in multi-service systems of care.
  • Previous WRAParound experiences a plus.

Conditions of Employment:

  • Occasional flexible work schedule required to meet individual client needs.
  • Must possess a valid CA Driver’s License, current auto insurance, and a clean DMV printout (no more than 3 points).
  • Must pass a clear background check with CACI, DOJ & FBI.
  • CPR and First Aid Certified.
  • TB Exam and Physical at the time of employment.

Driving Requirement:

  • Driving is a required function of this position. Employees must use a personal vehicle that meet all agency requirements, including maintaining liability insurance that meets or exceeds state requirements.
  • Employees are responsible for ensuring the timely renewal of their vehicle insurance policies and providing updated documentation to the agency to remain in good standing and compliant with position requirements.

Training Provided:

  • Comprehensive onboarding including CPR/First Aid, Crisis Communication/Pro-ACT, Trauma-Informed Care, Motivational Interviewing, and Collaborative Problem Solving.
