This role focuses on:
1-The Enhanced Care Management (ECM) benefit framework under CalAIM
2- patient assessment skills combined with the ability to deliver compassionate social work and mental health counseling. You will provide comprehensive, community-based care coordination and clinical support to high-need individuals, addressing both their complex medical needs and health-related social challenges.
- Clinical Supervision & Oversight: Provide clinical supervision to associate social workers, non-clinical Lead Care Managers, and peer support specialists.
- Documentation Review: Review, approve, and sign off on comprehensive assessments, individualized care plans, and treatment documentation to meet medical necessity.
- Care Coordination: Collaborate with interdisciplinary teams (ICT) to manage care for high-acuity, high-need populations, including field-based visits when necessary.
- Quality Assurance: Ensure compliance with Medi-Cal Managed Care Plans, and HIPAA regulations.
- Crisis Management: Support staff with crisis intervention, 5150 evaluation facilitation, and safety planning.
- Consultation: Serve as a clinical resource, providing training and coaching on trauma-informed care and evidence-based practices.
- Conduct thorough patient assessments, including intake interviews and diagnostic evaluations using ICD-10 and ICD coding systems.
- Maintain accurate medical documentation in compliance with HIPAA regulations and medical terminology standards within Electronic Medical Record (EMR) systems.
- Collaborate with multidisciplinary teams to coordinate case management, patient care plans, and community resources.
- Valid licensure as a Licensed Clinical Social Worker (LCSW) or equivalent in the relevant state or jurisdiction.
- Proven experience in psychotherapy modalities including motivational interviewing, behavioral health interventions, and crisis management.
- Familiarity with EMR systems for documentation and medical records management; knowledge of CPT coding is a plus.
- Knowledge of medical terminology related to mental health conditions such as PTSD, ICD coding (ICD-9/ICD-10), and psychopharmacology principles.
