Location: Long Beach
Schedule: Tuesday – Saturday | 9:00 AM – 5:00 PM
Pay Rate: $65,000 – $75,000 per year

Park Avenue Extended Care is seeking a compassionate and dedicated Full-Time Social Worker to join our interdisciplinary care team. This role offers the opportunity to provide essential psychosocial support to residents and families in a collaborative, resident-centered environment.

Key Responsibilities

• Assess psychosocial needs of newly admitted residents and families

• Provide social work services related to illness, adjustment, and placement

• Develop and implement individualized care plans

• Participate in interdisciplinary care plan meetings

• Advocate for resident and family concerns

• Conduct family meetings and therapeutic group sessions

• Ensure compliance with all federal, state, and facility documentation standards

• Assist with discharge planning and community resource coordination

• Collaborate with all departments to support quality outcomes

• Serve as a liaison between residents, families, and staff

Qualifications

• Master’s Degree in Social Work preferred

• Minimum 1 year of experience in a healthcare or medical setting

• Strong communication, organizational, and interpersonal skills

• Knowledge of community resources and discharge planning
