We are seeking a reliable and detail-oriented Home Care Coordinator to manage in-home care services throughout New York City. The coordinator will ensure all services are delivered in compliance with New York State Department of Health regulations while supporting clients enrolled in Medicaid, private pay, and Managed Long-Term Care plans.

This role requires strong scheduling skills, knowledge of NYC home care operations, and the ability to work in a fast-paced, high-volume environment.

Key Responsibilities

  • Coordinate and schedule Home Health Aides (HHAs) and Personal Care Aides (PCAs) across NYC boroughs
  • Manage cases involving Medicaid and MLTC-authorized hours
  • Maintain and update Plans of Care (POC) per physician and MLTC authorization
  • Serve as liaison between clients, families, caregivers, and MLTC case managers
  • Ensure compliance with HIPAA and NYS privacy laws
  • Monitor Electronic Visit Verification (EVV) for accuracy and compliance
  • Handle call-outs, emergency coverage, and last-minute schedule changes
  • Maintain accurate documentation for audits and New York State Department of Health inspections
  • Verify caregiver credentials and track in-service training requirements
  • Address client concerns, incident reports, and service issues promptly


Qualifications

  • High school diploma required; Associate’s or Bachelor’s degree preferred
  • 1+ year experience in a NYC home care agency (LHCSA, CHHA, or MLTC environment strongly preferred)
  • Strong knowledge of Medicaid home care services in NYC
  • Experience with scheduling high-volume cases and multiple borough coverage
  • Familiarity with EVV systems and home care software
  • Knowledge of UAS-NY is a plus
