Discharge Planner

Hours: 40 hours per week; Monday - Friday, 8:00 am to 4:30 pm. This is an on-site position and requires in-person attendance to perform the essential functions of the job.

Pay Rate: $28.85 to $29.13 per hour.

Education: Bachelor's degree in Psychology, Social Work, or Counseling required

As a Discharge Planner, you will:

  • Assess discharge needs of each person served and coordinate discharge planning activities to ensure continuing care supports are in place as needed
  • Collaborate with the treatment team to meet discharge goals
  • Assist with initial screening and sign in process to the program as well as provide crisis intervention, discharge planning groups and outreach activities to residents at the program on an as needed basis
  • Represent the program/CRF within the community by developing linkages and relationships with other services providers
  • Educate clients regarding supports and resources available

Required Skills and Qualifications

  • CPR/First Aid certification required and must be current throughout employment
  • Minimum of 4 years of experience in a behavioral health field
  • Extensive knowledge of San Diego resources for individuals with mental illness, substance use issues, and homelessness preferred
  • Ability to maintain calm demeanor in crisis situations is essential
  • This program is housed in a two-story building that does not have an elevator. Ability to navigate stairs is required.
  • Proficient computer skills, including MS Office
  • A valid California driver’s license in good standing, personal vehicle, proof of insurance and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position
  • Must be able to pass criminal/background clearance

Our Crisis Centers are Short Term Acute Residential Treatment (START) programs (Del Sur Crisis Center, Esperanza Crisis Center, Halcyon Crisis Center, Jary Barreto Crisis Center, New Vistas Crisis Center, Turning Point Crisis Center and Hawthorne Crisis Center) that provide intensive, community-based crisis residential services for adults throughout San Diego County. They deliver a unique alternative to psychiatric hospitalization to clients who are experiencing a psychiatric crisis of such magnitude so as not to be manageable on an outpatient basis. Their services are provided in a home-like environment that specializes in the psychosocial rehabilitation of each individual.

CRF staff members work together to meet the needs of San Diego County’s residents who are experiencing mental/behavioral health challenges.
