Clinical Coordinator

Overall Description:

Responsible for tracking and monitoring clinical staffing patterns to ensure staffing is at appropriate levels and in compliance with Medicare guidelines. Coordinates and communicates with Clinical Management to identify staffing needs and makes recommendations to alter staffing patterns to ensure patients and families are well cared for. Maintains effective communication between clinical staff, vendors, facilities, and families. This position also includes general clerical work to help support the office.


Responsibilities:

  • Partner with Clinical team to coordinate schedule in accordance with patient Plan of Care and Medicare Guidelines
  • Process workflow in EMR system
  • Answer phones and assist callers
  • Receives faxes and emails and address needs accordingly
  • Order Supplies and DME for current patients
  • Work with the Hospice Medical Directors for signatures on orders, death certificates, etc.
  • Additional job-related tasks as needed

Required Qualifications:

High school graduate or equivalent
Excellent customer service skills
3 years clerical experience, preferably in medical setting
Microsoft Excel, Microsoft Word, Outlook

Preferred Qualification:

Experience with staffing and scheduling systems
HomeCare HomeBase (EMR) knowledge
