Licensed Clinical Social Worker (LCSW) – Medical Social Worker

Home Health – South San Francisco / Bay Area, CA

Part‑time/Per Diem/PRN (1099 Contractor)

Pay: $75 - $80 per hour OR $95 - $105 per visit (based on experience) + Mileage Reimbursement

Schedule: Monday–Friday preferred with significant flexibility; occasional weekend visits as needed

What You’ll Do

• Conduct psychosocial assessments and develop individualized care plans
• Provide counseling and supportive guidance to patients and their families
• Facilitate advance care planning and education on advance directives
• Offer crisis intervention and bereavement support
• Coordinate with community resources and the interdisciplinary care team
• Educate patients and families on coping strategies and available support services
• Maintain timely and accurate documentation in the agency EMR
• Collaborate with physicians, nurses, and other team members

What You’ll Bring (Requirements)

• Active, unrestricted California LCSW license
• Master’s Degree in Social Work (MSW) from a CSWE-accredited program
• Minimum one (1) year of medical social work experience (home health, hospice, or hospital preferred)
• Thorough knowledge of Medicare home health guidelines
• Strong clinical assessment, counseling, and crisis management skills
• Excellent verbal and written communication skills
• Valid CA driver’s license, reliable transportation, and active auto insurance
• Current TB screening (within the last 12 months)
• Current Health Assessment and proof of immunizations/ titers: Hepatitis B (or signed declination), MMR, Varicella, and Tdap.
