Exciting Opportunity for Licensed Masters of Social Worker (LMSW) Job Description: - Join a dynamic healthcare team as a Licensed Masters of Social Worker (LMSW). - Shift Type: Day shifts, 4x8 hours (07:30 - 16:00). - Patient Ratio Experience: Varies; enjoy a manageable caseload of 18-24 patients, with weekends seeing up to 30 patients. - Experience Required: Minimum of 1 year in acute care, discharge planning, and care coordination. - Patient Age Groups: Adults and Geriatrics. - Daily Census: Engage with an average of 25 patients daily. - Float Required: Yes, willingness to float between like units is essential. - Weekend Coverage: Every other weekend. - Orientation Hours: 40 hours of comprehensive training. Qualifications: - Licensure: Current Washington state LMSW or LCSW license required. - Certifications: BLS certification is a must. - Charting System: Proficient in Epic is required. - Professional Knowledge & Skills: - Adult protective services, guardianship, and Power of Attorney. - Knowledge of cultural issues and communication problems (hearing, speech). - Biopsychosocial assessments and initial evaluations. - Case Management/Utilization Review: - Care coordination and discharge planning. - Plan of care and needs assessment. - Documentation of care delivery, plans, and goals. Interdisciplinary Collaboration: - Work closely with Physical Therapy, Interpretation Services, Social Services, Pharmacy, and Transportation teams. - Referrals to rehabilitation and long-term care facilities. Special Procedures/Unit Details: - Accepts LMSW or LCSW with a Master's Degree. - Must possess strong acute care experience in diverse cases—Med Surg, LLOS, ED.
