Position Overview
Little Sisters of the Poor, Sacred Heart Residence is seeking an experienced and compassionate Contract Social Worker Consultant to support the Social Services Designee within our long-term care community.
This consultant role will serve as a professional guide and resource to help strengthen social service processes, support regulatory compliance, and assist in addressing the psychosocial needs of residents and families. The ideal candidate will bring expertise in long-term care social services and a collaborative, mission-centered approach to mentoring staff.
Key Responsibilities
- Provide guidance and consultation to the Social Services Designee
- Assist with developing and strengthening social service practices and documentation
- Support compliance with state and federal long-term care regulations related to social services
- Offer consultation regarding resident psychosocial assessments, care planning, and discharge planning
- Serve as a resource for complex resident or family situations
- Assist with identifying appropriate community resources and referrals
- Collaborate with interdisciplinary team members including nursing, administration, pastoral care, and activities staff
- Provide recommendations for workflow improvements and best practices in resident-centered care
- Support staff education related to social service responsibilities and resident rights
- Promote compassionate, dignified care consistent with the mission of the organization
Qualifications
- Social work licensure (LSW or LCSW) required
- Master’s degree in Social Work (MSW) or related field preferred
- Minimum of 3–5 years of experience in long-term care, skilled nursing, or geriatric social services
- Strong knowledge of long-term care regulations, care planning, and psychosocial documentation
- Experience mentoring or supervising staff preferred
- Excellent communication, organizational, and interpersonal skills
- Ability to work collaboratively and provide supportive leadership
- Respect for and willingness to support the mission and values of the Little Sisters of the Poor
Preferred Qualifications
- Experience serving as a Social Services Director or consultant in a nursing home setting
- Familiarity with Alabama Department of Public Health and CMS long-term care requirements
- Experience with elder care, dementia support, family counseling, and crisis intervention
