Senior Director of Care Management & Population HealthAbout the Role

Asaar Medical is seeking an experienced and strategic leader to oversee Care Management and Population Health operations within our growing ACO REACH program. This role is ideal for a clinically strong leader with expertise in chronic disease management, value-based care, care coordination, and healthcare analytics.

The ideal candidate understands how to use data, workflows, and team leadership to improve patient outcomes, reduce avoidable utilization, close care gaps, and optimize performance across high-risk Medicare populations.

This position will work closely with executive leadership, physicians, care coordinators, and quality teams to strengthen and scale our care management infrastructure.

Responsibilities

• Lead and optimize Care Management and Care Coordination operations across the organization
• Develop disease management strategies for high-risk Medicare patients
• Use analytics and population health data to identify care gaps, utilization trends, and intervention opportunities
• Improve outcomes related to hospitalizations, readmissions, ED utilization, and quality metrics
• Oversee transitional care management, chronic care management, and patient engagement initiatives
• Supervise and mentor care coordinators, nurses, and support staff
• Collaborate with providers and leadership to improve workflow efficiency and patient outcomes
• Support CMS, ACO REACH, HEDIS, STAR, and value-based care performance initiatives

Qualifications

• RN, BSN, LCSW, MSN, MPH, or related healthcare leadership background preferred
• 5+ years of experience in Care Management, Population Health, ACO, Medicare Advantage, or Value-Based Care
• Strong knowledge of chronic disease management and high-risk patient populations
• Experience using healthcare analytics and population health reporting tools
• Proven leadership experience managing multidisciplinary care teams
• Familiarity with CMS quality programs, utilization management, and care coordination workflows
• Experience with ACO REACH or risk-bearing organizations strongly preferred

Pay: $51,552.71 - $75,000.00 per year
