If you are passionate about providing quality care, possess strong organizational skills, and thrive in a collaborative setting, ABL Health Care is the place for you.Position Overview:We are seeking an experienced, self-driven Full Time Medical Social Worker (MSW) to join our dedicated Home Health team and provide exceptional care throughout the region. As an Medical Social Worker (MSW), you will evaluate patients' health, create and implement care plans, and track their progress to maintain the highest standards of care. Whether you need assistance with a new situation, help communicating with a patient if you're running late, or someone to call in lab results, you'll always have reliable support when you're part of our team.We provide industry-leading tools to streamline your daily operations.Competitive compensation, including: Medical, Dental, Vision, 401(k), Life Insurance, and more. (Stipend For Full-Time Positions Only for health insurance)We also have Health Insurance options for Part-Time team members Bonuses based on quality metrics (NOT measured on volume of visits).Option for a company vehicle(based on availability for Full Time only).Company devices provided for ease of record-keeping, communication, and collaboration.QUALIFICATIONS:Must have a Masters Degree from a Social Work accredited by the Council on Social Work Education.One (1) year of Social Work experience in a health care setting.  Must be able to function in an independent fashion and have considerable maturity of judgment.DUTIES:Communicates significant information regarding patients with other members of a health care team and makes necessary recommendations and suggestions to improve the Plan of Care.Collaborates with the physician and other members of the health team to develop a Plan of Care.Assessment of psychological, emotional and/or economical factors affecting patient’s limitations and potential for and/or lack of improvementInitial evaluation visit, interim evaluations (as deemed necessary) and discharge evaluation from Social Services are required documentationAssessment of patients needs for long term care including home and family situation, exploring alternatives to in home care arrangement for placementCounsel patient, caregiver regarding long term planning and decision-making.Clinical notes should be written each visit and include progress, or lack of, specific plans, goals, anticipated length of service, referrals made and follow up on referralsIdentify high risk indicators, potentially endangering patients and provide intervention,  report situation to Case Managers and proper authoritiesProvide brief therapy to facilitate improved coping, adjustment, management, and compliance to medical regimenAttends staff meetings, participates in in-services, case conferences and Quality Assurance Performance Improvement activity as requiredPrepares clerical and progress notes and submits them in a timely manner to the officeThis job description should not be interpreted as an exhaustive list of all responsibilities, duties, or skills that may be required of the employee.
