About the role

The April Parker Foundation is seeking compassionate, detail-oriented Regional Care Managers to deliver In-Person Enhanced Care Management (ECM) and Community Supports (CS) services to Medi-Cal members with complex medical, behavioral, and social needs.

This is a Field-Based (Client Services) positionideal for professionals who value flexibility, autonomy, and meaningful impact. Regional Care Managers are assigned a territory/region and engage clients in-person to address physical health, behavioral health, and social determinants such as housing, food access, and transportation.

Regional Care Managers work directly with individuals experiencing housing insecurity, chronic illness, behavioral health challenges, and justice involvement, helping them navigate care systems, access resources, and achieve stability in health and housing.

IMPORTANT: This role requires in-person engagement, including at a minimum one in-person visit per member per month within your assigned region.

What you'll do

  • Conduct outreach, intake, and comprehensive assessments for ECM and CS members
  • Develop and implement individualized care plans
  • Coordinate medical, behavioral health, and social services, including:
  • Housing navigation and tenancy support
  • Food access and transportation resources
  • Provide in-person, telephonic, and virtual services based on member needs
  • Maintain accurate, timely documentation in APF systems
  • Collaborate with health plans, community partners, and internal multidisciplinary teams
  • Ensure compliance with program requirements
  • Support members in achieving long-term stability and self-sufficiency

How Compensation Works

We offer a transparent, performance-driven compensation model that rewards active engagement and in-person service delivery.

Base Pay

$25.00/hour for all hours worked. This is a full-time, hourly, non-exempt position, and all hours must be logged. (Base pay is approximately $4,000 per month, based on 40 hours worked per week)

In-Person Performance Incentive Pay (Additional Earnings)

$40.00 per member per month (PMPM) for each member who receives a qualified in-person ECM or CS service during the monthly service period. This incentive is the engine of the compensation model. Care Managers who show up in the field and serve their members earn significantly more, up to an additional $2,400 per month.

A Care Manager consistently serving a caseload of 40–60 active members can expect total monthly earnings of upto $6,400 per month, depending on hours worked and caseload size.

Reimbursements & Stipends

  • Mileage reimbursement at the IRS rate
  • $50/month phone stipend
  • Reimbursement for approved work-related expenses

Schedule

Monday–Friday, with occasional weekend and evening availability as required by member needs. The majority of scheduled hours are spent in the field conducting face-to-face member visits across your assigned territory. Daily field presence is expected; this is not a home-office or desk-based role.

Qualifications

Required

  • Minimum 2 years of experience in:
  • Case management
  • Care coordination
  • Social services or related field
  • Knowledge of:
  • Medi-Cal / CalAIM programs
  • Community resources
  • Social determinants of health
  • Strong:
  • Documentation and organizational skills
  • Communication and relationship-building skills
  • Valid California Driver’s License
  • Reliable transportation and auto insurance

Preferred

  • Bachelor’s degree in a related field
  • Community Health Worker (CHW) certification
  • California licensure: LCSW, LMFT, LPCC, RN, or similar
  • Bilingual: Spanish
