POSITION SUMMARY: Coordinate and provide case management services, ensure quality of care to the consumer population and assist each individual in maintaining his/her independent living environment.

ESSENTIAL ACCOUNTABILITIES

1. Responsible for the operation of the Supported Housing program and assigned caseload. Drive self and/or drive agency vehicle throughout boroughs to visit apartments routinely, to assess, observe and evaluate physical plan and residents functioning abilities and needs.

2. Act as a liaison with community health, mental health, educational, vocational and recreational programs.

3. Supervise and collect urine samples. Ensure procedure for accurate testing.

4. Provide linkage services with detoxification and rehabilitation treatment centers.

5. Assist consumers in the housing selection process including participation in the selection of adequate furnishings, working with real estate’s agents securing service from utility companies (telephone, electric), etc.

6. Facilitate sublease and rental payment agreement.

7. Facilitate housing subsidies, food stamps, other benefits and entitlements.

8. Assist consumers to link to community support services in identified areas of need.

9. Familiarize the consumer with his/her rights as a tenant and assist in tenant/landlord communication

10. In conjunction with consumer, develop individual support plan to address areas such as self-care (i.e., maintenance of sobriety), money management, medication management, daily living skills and symptom management

11. Maintain all required written documentation including consumer records and program records.

12. Identify maintenance and safety needs of assigned apartments.

13. Responsible for reporting and documentation of all incidents to immediate supervisor and/or the Director of MHS within 24 hours.

14. On-call 24 hours a day, seven (7) days per week to respond to emergencies.

15. Weekly supervision of assigned staff, to delegate tasks and provide professional direction and technical assistance.

16. Required to participate and attend meetings/trainings at various sites that require following different route of travel to worksite.

17. Able to lift/carry packages and boxes of supplies; as well as clients if necessary.

18. Ability to count, add, subtract, and manage money

19. Communicate information with all staff and supervisor(s) verbally and in writing for the purpose of giving instructions, providing feedback and adjusting work practices.

20. If deemed necessary must be able to participate in these programs at various sites and varying times. The ability to complete the following training offered and perform the techniques taught in each area a required function of the position; CPR and First Aid.

OTHER RESPONSIBILITIES

1. Participate in activities of other staff members in their absence or during periods of staff shortage.

2. Represent the agency at meetings, trainings not otherwise specified.

3. Ability to work flexible schedule as work schedules are subject to change.

4. This does not limit the assignment of duties or exclude the performance of other duties not mentioned, as assigned by supervisor.

5. Maintain confidentiality at all times.

Bachelor’s degree in Social Services preferred, experience working with mental health consumers with/without co-occurring addictive disorders. New York State driver’s license and access to a vehicle required.
