Job Overview
Superior Home Care Services, Inc. (SHCS) is excited to open up the position of Client Care Coordinator (CCC). The CCC works directly with clients to improve overall quality of life, and with caregivers to assist and mentor them as needed. Responsibilities include the following.
Responsibilities
- Client Assesments: Meet with new potential clients and assess / document their needs. This includes filling out a client assessment form and entering in all the case details into the system so that the client can be matched with the most appropriate caregiver.
- Case Starts / Caregiver Introductions: Ensure new cases are started properly and caregivers have all the necessary resources in the home to be successful. Introduce caregivers to new clients and ensure that they understand the case and the client’s needs, likes and dislikes.
- Client Enrichment: Planning and carrying out enrichment activities for clients to impove clients’ mental well-being. This can may include brain games, puzzles, arts and crafts, music or age / condition appropriate exercise.
- TLC Visits: Client wellness checks to ensure their needs are met and that they’re happy.
- Drop In Visits: Drop-in visits to assess caregiver performance. Provide mentoring and appropriate feedback to caregiver when needed.
- Mentor Caregivers: Mentor, assist and training caregivers as needed.
- Care Management: Manage and schedule client appointments, take clients to appointments, update / maintain notes of the appointment’s reasons and outcomes, communicate with client’s family members / concerned party regarding client’s condition and advocate on client’s behalf. This includes keeping SCHS management abreast of client and project statuses and entering notes into the system.
- On Call Responsibilities: When needed, filling in for caregiving shifts during last minute call offs until shift can be restaffed by the scheduler.
Qualifications
- Must be able to read, write, and speak English.
- Must be at least 18 years of age.
- Must be able to lift at least 65 pounds.
- Must demonstrate and be knowledgeable and comfortable with performing basic caregiving tasks such as assisting a client using a gate belt, using a hoyer lift, transfering a client with limited ambulation, bathing, dressing, and toileting.
- Required to drive and must possess a valid driver’s license, registration and auto insurance.
- Must have reliable transport and be willing to drive to multiple client locations throughout San Diego county daily.
- Attention to detail and organized to include daily shared calendar updated and client notes updated.
- High school diploma and professional certification (CNA, LVN etc) desirable.
- Must have 2 years or more experience working with the elderly.
- Must possess a high level of customer empathy, and emotional intelligence when dealing with families in a crisis.
- Must possess the ability to deal tactfully and respectfully with staff coworkers, clients and family members.
- Must be highly organized and able to self-manage time in order to meet appointments, and prioritize a constantly changing schedule as needs arise in the field with our clients.
- Must be computer literate, able to use text and access our online programs in the field.
- Must pass background checks performed by Company or Company’s third-party vendor including fingerprinting, and criminal background check, and TB clearance to become HCA registered.
- Must be willing to transport SHCS clients and / or Caregivers if the situation calls for this assistance.
- Must possess the ability exercise sound judgment and make independent decisions when emergency circumstances warrant it.
- Must demonstrate knowledge of the safe use and care of supplies and equipment.
Job Type: Full-time
Pay: From $25.00 per hour
